For Out-Patient Treatment
  1. Completely filled out Attending Physician’s Statement for Out-Patient Treatment.
  2. Original official receipts with payment details.
  3. If with surgical operation, photocopy of the operative report.
  4. Photocopy of doctor’s presentation of medication required for treatment.
  5. Photocopy of doctor’s request for laboratory procedure.
  6. Photocopy of laboratory results.
  7. If accident related, original police report/ accident report.

For In-Patient Treatment
  1. Original and full medical report (pertaining to the confinement) from the hospital or Attending physician/s (e.g. clinical abstract/ admitting history, discharge summary, etc).
  2. Original official receipts with payment details.
  3. If with surgical operation, photocopy of the operative report.
  4. Photocopy of doctor’s prescription for home medication.
  5. If accident related, original police report/accident report.
  6. Photocopy of laboratory results.
  7. Photocopy of doctor’s prescription for home medication.
  8. Completely filled out attending Physician’s statement for In-patient treatment.
  9. Personal Accident Benefit
    1. Doctor’s official and original written diagnosis.
    2. Full medical report establishing the loss of limb or loss of sight or total and permanent disability.
    3. In case of death, a photocopy of the death certificate and the relevant coroner’s report.
    4. If the accident was reported to the police authorities, original police report/ accident accident report.
    5. Completely filled out Attending Physician’s Statement for In-Patient Treatment.
Baggage Loss or Damage Benefit.
  1. Original baggage irregularity report issued by the airline/carrier in case the loss or damage occurred while such baggage was under their custody.
  2. Certification from carrier, hotel or any other party, or any proof satisfactory to the Company that the loss or damage to the baggage was not indemnified, or Indemnified, or if settlement was made certification specifying amount settled.
  3. For other circumstances, an original police report for such loss or damage.
  4. Original official receipts for the purchase of the lost or damaged bag. If original official receipts are not available submit an affidavit of lost official receipt. Details of the official Receipts such as date of purchase, brand, model, and type of lost or damage bag should be specified on the affidavit.
  5. Or, if already repaired, original official receipt for the cost repair with corresponding details.
  6. Original official receipt for the purchase of the lost or damaged articles within the the baggage (e.g. clothing, prescribed medicines, bag, footwear, etc) If original official receipt are not available, submit an affidavit of lost Official receipt. Details of the official receipt such as amount, brand and type of lost or damage articles should be specified on the affidavit.
